You may register for the NCCOA Holiday Social Fundraiser by printing the registration form or by completing the on-line registration form below. Registration Fee payments can be submitted in one of two ways:

Event sponsorships are available and items are
being accepted for raffle prizes. Contact cwalker@transitionslifecare.org
for more information or to secure a sponsorship and/or to
provide a raffle prize.
If you are unable to attend but
wish to contribute to our fundraising event, please use the
Donate button under the Additional Donations Section.
